Regulation for Registration of Medical Devices Article 31
The following documents shall be attached when applying for re-issuance of a lost or damaged medical device permit license:1.Application form for change in medical device permit license.2.The original copy of the original permit license must be attached when applying for re-issuance of a damaged permit.3.In the event of the original license being lost, an affidavit stating that the original permit license indeed being lost must be attached.4.One original and one photocopy of the medical device registration application form.

申請醫療器材許可證遺失補發或污損換發，應檢附下列資料：
一、藥物變更登記申請書。
二、如係申請許可證污損換發者，應檢附原許可證正本。
三、如係遺失者，應檢附聲明原許可證確係遺失之切結書。
四、醫療器材查驗登記申請書正、副本各一份。
