Regulations for Issuance of License of Specific Purpose Cosmetics Article 23
Applying for an extension of the term of validity of the license of specific purpose cosmetics per Paragraph 5 of Article 5 of the Act, the applicant shall submit an application with the following documents, and pay the fee to the central competent authority:1. The original license.2. The copy of the company or business registration certificate.3. The Authorization letter issued within the past two years except for domestic manufacturing.

依本法第五條第五項規定申請特定用途化粧品許可證有效期間展延者，應填具申請書，檢附下列文件、資料，並繳納費用，向中央主管機關提出：
一、原許可證。

三、二年內出具之授權書；國產者，免附。
