Regulations Governing Accreditation and Outsourced Accreditation Management of Cosmetic Testing Institutions Article 8
The accreditation certificate shall contain the following information:1. The title of the testing institution.2. Thetitle and address of the laboratory, and the full name of the laboratory head.3. Test items as accredited, test method, test scope, and the report signatory.4. Year, month, day, and serial number of the accreditation certificate.5. Validity period of the accreditation certificate.The testing institution shall display the accreditation certificate in a highly visible place of the premises.

認證證明書應載明下列事項：
一、檢驗機構名稱。
二、實驗室名稱、地址及負責人姓名。
三、經認證之檢驗項目、檢驗方法、檢驗範圍及報告簽署人。
四、認證證明書核發之年、月、日及認證編號。
五、認證有效期間。
檢驗機構應將認證證明書揭示於該機構明顯處所。
