Standards of Service Fees for Application Review of Human Organ Bank Article 3
The Service fees for application review under the preceding Article are listed as follows :1. Permit application(1) NT$120,000 for each application.(2) With 2 or more applications from the same institution are NT$80,000 for the second application and thereafter.2. Application for permit extension:(1) NT$80,000 for each application of permit extension.(2) With 2 or more application of permit extension from the same institution are NT$50,000 for the second application and thereafter.3. Application for changes of permit registration items:The changes of name of the institution, address, representative or responsible person, medical supervisor or quality control supervisor are NT$5,000 for each application.4. Application for the inspection on human organ bank located in foreign countries:(1) NT$120,000 for each application review and inspection.(2) The on-site inspection fee of the inspector shall be charged based on the Guidance for Reimbursement for Overseas Traveling Expenses.

前條審查費收取費額如下：
一、申請許可：
（一）每一申請案收取新臺幣十二萬元。
（二）同一機構同時申請許可達二件以上者，第二件起每件收取新臺幣八萬元。
二、申請許可效期展延：
（一）每一申請案收取新臺幣八萬元。
（二）同一機構同時申請許可效期展延達二件以上者，第二件起每件收取新臺幣五萬元。
三、申請許可證明記載事項變更：保存庫申請機構名稱、地址、代表人或負責人、醫學主管或品質主管變更，每件收取新臺幣五千元。
四、申請實施國外保存庫檢查：
（一）每一案檢查之審查費收取新臺幣十二萬元。
（二）查核人員臨場費，應比照國外出差旅費報支要點所定標準辦理。
