Regulations for Management of Assistant Dentists Article 3

In case of damage to or loss of their assistant dentist certificates, assistant dentists shall apply to the central competent authority for replacement or reissuance thereof.Any person who applies for replacement due to damage to their certificate, as set forth in the preceding paragraph, shall return the original certificate. Any person who applies for reissuance due to loss of their certificate shall sign an affidavit, and shall return the lost certificate immediately after it is recovered.
