Hearing Specialists Act Article 13
When conducting business, hearing specialists shall prepare their own record, sign or affix seal thereon and note the date (year, month and day) and the following particulars: 1. Content of doctor’s consultation, order or referral. 2. Course of services rendered. 3. Other required matters.The record mentioned in the preceding paragraph shall be kept on file together with the patient’s name, sex, date of birth, National ID Card No. and address.

聽力師執行業務時，應親自製作紀錄，簽名或蓋章及加註執行年、月、日，並載明下列事項：
一、醫師照會、醫囑內容或轉介事項。
二、執行業務之情形。
三、其他依規定應載明之事項。
前項紀錄應併同個案當事人之姓名、性別、出生年月日、國民身分證統一編號及地址保存。
