Physicians Act Article 13
When issuing prescriptions, physicians shall clearly state the following on the signed or sealed prescription slip: 1. physicians full name 2. patient name, age, name of medication, dosage, amount, use and year, month and date prescription was issued

醫師處方時，應於處方箋載明下列事項，並簽名或蓋章：
一、醫師姓名。
二、病人姓名、年齡、藥名、劑量、數量、用法及處方年、月、日。
