The Enforcement Rules of Dietitian Act Article 3
Where the dietitian certificate is destroyed or lost, the dietitian shall submit a completed application form together with the required processing fee to the competent central authority to process the reissuance of a new certificate.Where the dietitian certificate is damaged, the dietitian shall submit a completed application form and the original certificate together with the required processing fee to the competent central authority to process the certificate replacement.

營養師證書滅失或遺失者，應填具申請書，並檢附證書費，向中央主管機關申請補發。
