Those who lose their respiratory therapist certificate shall fill in an application form and submit it, along with the fee for the certificate, to the central competent authority for re-issuance of the certificate.Those who damage their respiratory therapist certificate shall fill in an application form and submit it, along with the fee for the certificate and their original certificate, to the central competent authority for re-issuance of the certificate.

呼吸治療師證書滅失或遺失者，應填具申請書，並繳納證書費，向中央主管機關申請補發。
呼吸治療師證書損壞者，應填具申請書，並繳納證書費，連同原證書，向中央主管機關申請換發。
