Those who have passed the practitioner examination may apply to the central competent authority for issuance of a practitioner certificate.The following items shall be indicated on thecertificate:1.Specialty;2.Certificate number;3.Name and gender;4.ID number;5.Date of birth;6.Period of validity of the certificate;7.Date of issue.
中文原文 專科護理師分科及甄審辦法第15條
經專科護理師甄審合格者，得向中央主管機關申請發給專科護理師證書。
前項證書，應記載下列事項：
一、專科別。
二、證書字號。
三、姓名、性別。
四、國民身分證統一編號。
五、出生年月日。
六、證書有效期間。
七、發證日期。
