If one applies for supplying again or renewing the certificate and license granted by this Act because of loss or extermination, he shall fill in application, prepare the charge of certificate and then send to the central competent authority to transact it. If one applies for supplying again or renewing the certificate and license granted by this Act because of defilement, he shall fill in application, prepare the charge of certificate and the original certificate and then send to the central competent authority to transact it.

社會工作師證書滅失或遺失者，應填具申請書，並繳納證書費，向中央主管機關申請補發。
社會工作師證書損壞者，應填具申請書，並繳納證書費，連同原證書，向中央主管機關申請換發。
