The forms of the statements and documents referred to in subparagraphs 1 and 4, Paragraph 1 of Article 11, Paragraph 1 of Article 14, and Paragraph 1 of Article 37 shall be prescribed by the competent authority.The application/report forms referred to in Paragraph 2 of Article 16, Article 33 and Paragraph 1 of Article 40 shall be separately prescribed by the BLI.
