Enforcement Rules of the Employment Insurance Act Article 13
When an insured person claims unemployment benefit according to Subparagraph 1, Paragraph 1, Article 11 of the Act should prepare the following documents:1. Unemployment status examination (confirmation), unemployment benefits application form and benefit receipt.2. Employment separation certificate or proof of fix term contract.3. National identification card or the copy of other identification proof.4. A copy of the insured's personal bank passbook with any domestic financial institution. This is not required, however, if the account for payment is the same as the one for vocational training living allowances.5. For those with physical or mental disabilities, a certificate of physical or mental disability issued by social welfare administrative authority should also be enclosed.6. If there is any dependent, the following documents shall also be enclosed:(1) A copy of the household registration book of the dependents or the copy of other identification proof documents.(2) For those whose children are physically or mentally disabled, a certificate of physical or mental disability issued by social welfare administrative authority should also be enclosed.7.Foreign professionals, foreign specialist professionals, or foreign senior professionals shall additionally enclose a copy of a valid permanent residency document.

被保險人依本法第十一條第一項第一款規定請領失業給付者，應備具下列書件：
一、失業（再）認定、失業給付申請書及給付收據。
二、離職證明書或定期契約證明文件。
三、國民身分證或其他身分證明文件影本。
四、被保險人本人名義之國內金融機構存摺影本。
但匯款帳戶與其請領職業訓練生活津貼之帳戶相同者，免附。
五、身心障礙者，另檢附社政主管機關核發之身心障礙證明。
六、有扶養眷屬者，另檢附下列證明文件：
（一）受扶養眷屬之戶口名簿影本或其他身分證明文件影本。
（二）受扶養之子女為身心障礙者，另檢附社政主管機關核發之身心障礙證明。
七、外國專業人才、外國特定專業人才或外國高級專業人才，另檢附有效之永久居留證明文件影本。
