Enforcement Rules of the Labor Insurance Act Article 82
When the insured persons apply for funeral grants referred to in Article 62 of the Act, the following documents must be prepared:1.Application forms for funeral grants and Receipt of payments.2.Death certificate, public prosecutor's autopsy certificate, or death declaration ruling.3.Copies of valid household registration form with the registration of death listed and the National I.D. of the insured person or a copy of the household registration form.Those who have completed the registration of death may submit only the documents listed in subparagraph 1 of the previous paragraph.
中文原文 勞工保險條例施行細則第82條
被保險人依本條例第六十二條規定請領喪葬津貼者，應備下列書件：
一、喪葬津貼申請書及給付收據。
二、死亡證明書、檢察官相驗屍體證明書或死亡宣告裁定書。
三、載有死亡登記之戶口名簿影本，及被保險人身分證或戶口名簿影本。
已辦理完成死亡登記者，得僅附前項第一款所定文件。
一、喪葬津貼申請書及給付收據。
二、死亡證明書、檢察官相驗屍體證明書或死亡宣告裁定書。
三、載有死亡登記之戶口名簿影本，及被保險人身分證或戶口名簿影本。
已辦理完成死亡登記者，得僅附前項第一款所定文件。
