Applications for aid for unfair labor practice adjudication representation fees shall be presented within 30 days after the adjudication application is filed and the following documents shall be submitted:1.The application2.A photocopy of the Unfair Labor Practice Adjudication Application3.The financial statement of the applicant and related proof documents4.An affidavit stating that no aid from other government agencies has been receivedApplicants failing to submit all the documents specified in the preceding paragraph shall be notified to make corrections within a given period. The application from those failing to make corrections within the given period shall be rejected.

不當勞動行為裁決代理酬金扶助之申請，應於提起裁決之日起三十日內為之，並應檢具下列文件：
一、申請書。
二、不當勞動行為裁決申請書影本。
三、申請人之資力狀況及相關釋明文件。
四、未獲其他政府機關扶助之切結書。
前項所應檢附之文件有欠缺時，經通知限期補正，屆期未補正者，不予受理。
