Regulations on Import and Export of Endangered Species of Wild Fauna, Flora and Related Products Article 18
If exporter lost the permit issued by the TITA, during the shipping and applied for de-registration as Article 16 stipulates, shall submit the following documents to the TITA for re-issuance:1.A complete CITES export permit, re-export permit, or other permit application;2.Documentary explanation of loss of the permit;3.Export declaration or other relevant documents issued by the exporting customs;4.Proof of the responsible shipping firms;5.Other documents as specified.

出口人於貨品運輸途中遺失貿易署核發之華盛頓公約許可證或其他證明書，依第十六條規定申請註銷後，得檢附下列文件向貿易署申請補發：
一、華盛頓公約出口許可證、華盛頓公約再出口許可證或華盛頓公約其他證明書申請書。
二、遺失書面說明。
三、出口地海關核發之出口報單證明聯或其他相關文件。
四、運輸承攬業者之證明。
五、其他依規定應檢附之文件。
