Integrated Circuit Layout Protection Act Article 11
The application required in the preceding Article shall describe the following:1. Name, nationality, and domicile or residence of the applicant; in the case of a legal entity, its name, office address, and the name of its representative;2. Name, nationality, and domicile or residence of the creator; in the case of a legal entity, its name, office address, and the name of its representative;3. Title and date of creation; and4. Day, month, and year of the first commercial exploitation if commercial exploitation occurred prior to the filing date.

前條規定之申請書應載明左列事項：
一、申請人姓名、國籍、住居所；如為法人，其名稱、事務所及其代表人姓名。
二、創作人姓名、國籍、住居所；如為法人，其名稱、事務所及其代表人姓名。
三、創作名稱及創作日。
四、申請日前曾商業利用者，其首次商業利用之年、月、日。
