When filing a patent application for design, the application form shall specify the following items:1. title of design;2. name and nationality of the designer; and3. name, nationality, and domicile/residence or business establishment of the applicant; the name of the representative, if any; and4. name and business establishment of the agent, if an agent is designated;Where the priority prescribed under Paragraph 1, Article 28 applicable mutatis mutandis under Paragraph 1, Article 142 of the Act is claimed, a statement thereof shall be made at the time of filing.When filing a patent application for derivative design, the application number of the original patent application for design shall be specified in the request form, in addition to the items stated in the preceding two paragraphs.

申請設計專利者，其申請書應載明下列事項：
一、設計名稱。
二、設計人姓名、國籍。
三、申請人姓名或名稱、國籍、住居所或營業所；有代表人者，並應載明代表人姓名。
四、委任代理人者，其姓名、事務所。
有主張本法第一百四十二條第一項準用第二十八條第一項規定之優先權者，應於申請時敘明之。
申請衍生設計專利者，除前二項規定事項外，並應於申請書載明原設計申請案號。
