When filing a patent application for invention, the application form shall specify the following items:1. title of invention;2. name and nationality of the inventor;3. name, nationality, and domicile/residence or business establishment of the applicant; name of the representative, if any ; and4. name and business establishment of the agent, if an agent is designated.Under any of the following circumstances, a statement shall be made at the time of filing:1. where the priority prescribed under Paragraph 1, Article 28 of the Act is claimed;2. where the priority prescribed under Paragraph 1, Article 30 of the Act is claimed; and3. where the declaration is made in respect of a patent application for invention and a patent application for utility model filed for the same creation on the same date as prescribed under Paragraph 1, Article 32 of the Act.

申請發明專利者，其申請書應載明下列事項：
一、發明名稱。
二、發明人姓名、國籍。
三、申請人姓名或名稱、國籍、住居所或營業所；有代表人者，並應載明代表人姓名。
四、委任代理人者，其姓名、事務所。
有下列情事之一，並應於申請時敘明之：
一、主張本法第二十八條第一項規定之優先權者。
二、主張本法第三十條第一項規定之優先權者。
三、聲明本法第三十二條第一項規定之同一人於同日分別申請發明專利及新型專利者。
