Detention Act Article 88
Where a complaint is filed in a written format, the complainant shall fill out a complaint form, state thereon the following information, and then sign or affix a seal on the form:1. Name of the complainant; the name and address of the appointed agent or assistant, if applicable.2. Facts of the complaint and the time of occurrence.3. Reasons for filing the complaint.4. Date of the complaint.Where a complaint is filed verbally, a complaint form must be filled out by a detention center personnel on behalf of the complainant. After the complaint form is read to the complainant or reviewed by the complainant to confirm its contents, the complaint form shall be signed or sealed by the complainant.

以書面提起申訴者，應填具申訴書，並載明下列事項，由申訴人簽名或捺印：
一、申訴人之姓名。
有委任代理人或輔佐人者，其姓名、住居所。
二、申訴事實及發生時間。
三、申訴理由。
四、申訴年、月、日。
申訴人以言詞提起申訴者，由看守所人員代為填具申訴書，經向申訴人朗讀或使其閱覽，確認內容無誤後，交其簽名或捺印。
