Prison Act Article 124
A petition form must be filled out to file a petition. The petition shall provide the following information and sign or affix a seal on the form:1. Name of the petitioner; the name and address of the appointed agent or assistant, if applicable;2. Facts of the petition;3. Reasons for filing the petition;4. Date of the petition.
