Regulations Governing the Use of Weapons in Prisons and Detention Centers Article 11
If the use of weapon by an Officer causes death or serious injury, the authority shall immediately report to the supervisory authority.The Weapon Use Report under Paragraph 3 of the preceding article shall be submitted to the supervisory authority every month.
