The Anti-Corruption Informant Rewards and Protection Regulation Article 9
The corruption and malfeasance case by report shall be in written or oral manner.A written report shall specify the following items and shall be signed, sealed or fingerprinted by the informant:1.The Informant’s name, gender, date of birth, identification card No., address, residence or agency that he renders service, school, group and the party’s name who was reported or other characteristics for identification.2. Fact of corruption and malfeasance.3. Evidence.An oral report which specifies in detail shall be entered into records by the report-accepting authority to be signed, sealed or fingerprinted by the Informant, while in the case reported by phone call specifying in detail, the authority shall notify the Informant to make records at a designated place.

檢舉貪污瀆職案件，應以書面或言詞為之。
以書面檢舉者，應記載下列事項，由檢舉人簽名、蓋章或按指紋：
一、檢舉人姓名、性別、出生年月日、身分證明文件字號、住所、居所或服務機關、學校、團體，及被檢舉人之姓名或其他足資辨別之特徵。
二、貪污瀆職事實。
三、證據資料。
以言詞檢舉且內容具體者，由受理檢舉機關作成筆錄，交檢舉人閱覽後簽名、蓋章或按指印。
其以電話檢舉且內容具體者，受理檢舉機關應通知檢舉人到達指定處所製作筆錄。
