The conclusion, modification of an adult guardianship agreement shall be made in the notarization made by the notary public. The notary public shall give a written notice to the court for the place of the domicile of the ward within seven days after the agreement notarized.The notarization, as specified in the preceding paragraph, shall be performed by a notary public in the presence of both parties with their intentions.An adult guardianship agreement becomes effective from the date of the declaration of commencement of guardianship.

意定監護契約之訂立或變更，應由公證人作成公證書始為成立。
公證人作成公證書後七日內，以書面通知本人住所地之法院。
前項公證，應有本人及受任人在場，向公證人表明其合意，始得為之。
意定監護契約於本人受監護宣告時，發生效力。
