Regulations for Petitions against Discrimination against People Residing in the Taiwan Area Article 4
The petition shall contain the following items along with the signature(s) or seal(s) of the petitioner and/or his/her proxy:The petitioner’s name, date of birth, ID document number, residence address, and telephone number;The proxy’s name, date of birth, ID document number, residence address, and telephone number if a legal proxy acts on behalf of the petitioner;The identity of the defendant;Demands of the petition;Material facts and reasons of the petition;Documentary evidence if available;The date of the petition.

申訴應具申訴書，載明下列事項，由申訴人或其代理人簽名或蓋章：
一、申訴人之姓名、出生年月日、身分證明文件字號、住居所、電話。
二、有申訴代理人者，其姓名、出生年月日、身分證明文件字號、住居所、電話。
三、被申訴人。
四、申訴請求事項。
五、申訴之事實及理由。
六、證據。
但無法提供者，免附。
七、申訴之年、月、日。
