Mortuary Service Administration Act Article 33
Public cemeteries and ashes (skeletons) storage facilities shall have a permanently preserved register with the following items:1.Number of tomb bases or columbarium2.Burial or storage dates3.The name, sex, place of birth, date of birth and death date of the buried body.4.The name of the owner or depositor, his/her national identity card number, place of birth, address and relationship to the buried body.5.Other to-be-recorded matters specified by the competent authority for approval

公墓、骨灰（骸）存放設施應設置登記簿永久保存，並登載下列事項：
一、墓基或骨灰（骸）存放單位編號。
二、營葬或存放日期。
三、受葬者之姓名、性別、出生地及生死年月日。
四、墓主或存放者之姓名、國民身分證統一編號、出生地、住址與通訊處及其與受葬者之關係。
五、其他經主管機關指定應記載之事項。
