Enforcement Rules of Lobbying Act Article 4
The record letter mentioned in the preceding article shall record the following items：1.Natural person：(1) name, sex/gender, date of birth, and the number of National ID card , valid passport, or the personal ID card issued by foreign governments.(2) the number of the certificate passing the Special Qualification Examination for Professional and Technical Personnel.(3) the number of the certificate of executing business. (4) other items designated by the competent institute. 2.Legal person for profit:(1) the title and the address of the main office.(2) the name of the representative and the number of National ID card , valid passport, or the personal ID card issued by foreign governments. (3) the number of the registration certificate of the legal person.(4) other items designated by the competent institute.

前條備案函，應載明下列事項：
一、自然人：
（一）姓名、性別、出生年月日及國民身分證統一編號、有效護照號碼或外國政府核發之身分證明文件號碼。
（二）專門職業及技術人員考試及格證書字號。
（三）執業證明文件字號。
（四）其他經主管機關指定之事項。
二、營利法人：
（一）名稱及主事務所所在地。
（二）負責人姓名及國民身分證統一編號、有效護照號碼或外國政府核發之身分證明文件號碼。
（三）法人登記證明文件統一編號。
（四）其他經主管機關指定之事項。

（一）姓名、性別、出生年月日及國民身分證統一編號、有效護照號碼或外國政府核發之身分證明文件號碼。
