Applications for awarding a Professional Medal under these Regulations shall be submitted by the respective competent authorities or associations at all levels, accompanied by a completed Award Application Fact Form and relevant supporting documents. After review and approval by the Ministry and determined by the Minister, the award shall be awarded in a public ceremony.The format of the Award Application Fact Form referred to in the preceding paragraph is shown in Appendix II.Appendix II.odt
