Petitions Act Article 5
A people's petition shall have a petition statement, which includes the following matters, and shall be signed and sealed by the petitioner or petition group and its person in charge:1. The name, gender, age, place of origin, occupation, and address of the petitioner; if the petitioner is a group, the name, address, and person in charge of the group.2. The facts, reasons and wishes on which the petition is based.3. The authority shall accept the petition.4. The year, month, and date of the Republic of China.

人民請願應備具請願書，載明左列事項，由請願人或請願團體及其負責人簽章：
一、請願人之姓名、性別、年齡、籍貫、職業、住址；請願人為團體時，其團體之名稱、地址及其負責人。
二、請願所基之事實、理由及其願望。
三、受理請願之機關。
四、中華民國年、月、日。
